Call (203) 688-2544 between the hours of 8am – 5pm, Monday-Friday to make an appointment. Please have your insurance information available. As a courtesy, please give 24 hour notice for cancellations.
What to Bring
WHAT SHOULD I BRING TO MY APPOINTMENT?
You should bring your completed forms, copay if required (cash, check, MasterCard, Visa), insurance card and a valid government-issued photo ID (i.e. driver’s license, passport), your referral, a list of current medications, and full payment if you do not have an insurance carrier. If you have prior reports or X-Rays from another physician about your current problem, please bring those as well.
Important: If you are required by your insurance plan to obtain a referral from your primary care physician, this must obtained prior to your visit or you will not be seen.
